👋 Hey directors — we need your eyes on this

Leadership greenlit the case management system. Before we lock down what NYCID will track for every participant, we need your input on what your program actually needs to measure. So we did two things at once with this form:

  • Pretend you're entering a real participant. Pick someone you know well — pretend you're their counselor doing intake. Fill out what you'd want captured.
  • Tell us what's missing or wrong. The last step is a feedback section. Flag anything you'd add, remove, or word differently for your program.

Takes about 10–12 minutes. You're not committing to anything — this is the conversation, not the final form. Bonus: you'll get a feel for what the system is going to be like for your staff.

Note on data: this is a demo. Nothing you enter is saved to a real database — at the end you'll see a summary you can copy and email to me. The real system will save securely to Firebase with per-user access controls.

⚠️ Sensitive question framing: The next question is about violence exposure. We ask because programs need to know for safety planning and trauma-informed services — not for surveillance. Always optional. "Prefer not to say" is a valid answer that does not affect services.
📝Director note: Programs serving justice-involved youth, domestic violence survivors, or specific health populations probably need more here. What additional barriers should we capture? Should violence exposure be more granular, or removed entirely for some programs?
