👋 Why we're asking

We're building NYCID's first real case management system to replace our spreadsheets. The Intake Form covers what we believe every program needs to know about every participant. This survey is about what YOUR program needs that's NOT in the standard intake.

Your answers become the actual specification for what we build. The more specific, the better. Takes about 10 minutes — fill it out, then export and email back to Michael.

Tip: open the Intake Form in another tab as a reference while you fill this out, so you can see exactly what fields are already covered.

02Funder & compliance reporting
What outside entities require data from your program — DYCD, DOE, foundations, etc.
03Standard intake review
Looking at the standard intake form: are there fields that don't apply to your program, or fields that are missing?
04Program-specific fields you track
Add every field your program collects beyond the standard intake. These become the program-specific section participants see when they're enrolled in your program.
Example — SYEP would add fields like: "Worksite name", "Worksite address", "Hourly wage", "Supervisor name", "Hours worked this week", "Direct deposit set up", "Performance evaluation date".
Example — Career Ready would add: "Cohort number", "Certifications earned", "Resume upload date", "Interview practice completed", "Mock interview score".

05Activities & interactions you log
When a staff member works with a participant, what kinds of activities do they log? (At Concord, examples include: Counseling Session, Outreach Attempt, Post-Secondary Planning, Work-Based Learning.)
06Milestones & outcomes that matter
What participant outcomes does your program track? These are the "did we move the needle?" markers — the things you'd want on the dashboard.
07Today's pain points
What about your current data tracking is broken or frustrating? (This is where the new system pays off.)
08Who should see what
Different staff need different levels of access. This shapes our security rules.
